Memberships are only for patients without insurance. If you do not make your payments on time, your membership will expire. If re-applying for an expired membership plan, a new activation fee is required. Missed payments and reactivated memberships automatically reset any accrued discount benefit to first year benefit levels. Discounts previously mentioned do NOT apply to patients with insurance, other discount plans, or to patients paying with any sort of financing plan, such as Care Credit. Your membership is only valid while you are an active patient of Stanton Park Dental. You must have an account in good standing and a good standing relationship with the doctor. Should the doctor-patient relationship trust be breached, or for any other reason deemed appropriate by Stanton Park Dental, the doctors at Stanton Park Dental reserve the right to cancel any membership. Involved patients will be referred to an outside dental practice in such an example. Lastly, memberships may be subject to other conditions not mentioned here. Memberships are non-refundable.
